Informed consent indicates your awareness of sufficient information to allow you to make an informed personal choice concerning your child\u2019s oral health treatment after considering the risks, benefits, and alternatives.<\/p><p data-start=\"797\" data-end=\"923\">Please read this form carefully and ask about anything you do not understand. We will be pleased to further explain it to you.<\/p><h2 data-section-id=\"13jd9av\" data-start=\"925\" data-end=\"962\">Authorization for Dental Treatment<\/h2><p data-start=\"964\" data-end=\"1233\">I hereby authorize and direct the Doctors at PEDIATRIC DENTAL CENTER OF GARDENS INC, assisted by dental auxiliaries of their choice, to perform upon my child (or legal ward for whom I am empowered to consent) the following dental treatment or oral surgery procedure(s):<\/p><ul data-start=\"1235\" data-end=\"2109\"><li data-section-id=\"27gxs7\" data-start=\"1235\" data-end=\"1302\">Examination and radiographs (X-rays) as determined by the dentist<\/li><li data-section-id=\"5sqz2j\" data-start=\"1303\" data-end=\"1362\">Cleaning of the teeth and application of topical fluoride<\/li><li data-section-id=\"1hmngyt\" data-start=\"1363\" data-end=\"1430\">Application of \u201csealants\u201d to the fissures or grooves of the teeth<\/li><li data-section-id=\"13ptc27\" data-start=\"1431\" data-end=\"1468\">Administration of local anesthetics<\/li><li data-section-id=\"13a0b85\" data-start=\"1469\" data-end=\"1569\">Treatment of diseased or injured teeth with dental restorations (fillings, crowns and pulpotomies)<\/li><li data-section-id=\"1nvbf24\" data-start=\"1570\" data-end=\"1614\">Removal (extractions) of one or more teeth<\/li><li data-section-id=\"ssct24\" data-start=\"1615\" data-end=\"1681\">Treatment of diseased or injured oral tissues (hard and\/or soft)<\/li><li data-section-id=\"wgx53d\" data-start=\"1682\" data-end=\"1760\">Replacement of missing teeth with space maintainers and\/or dental prosthesis<\/li><li data-section-id=\"181d0c2\" data-start=\"1761\" data-end=\"1852\">Treatment of malposed (\u201ccrooked\u201d) teeth and\/or oral developmental or growth abnormalities<\/li><li data-section-id=\"16kq6pp\" data-start=\"1853\" data-end=\"1924\">Use of behavioral guidance techniques and\/or protective stabilization<\/li><li data-section-id=\"1sztswk\" data-start=\"1925\" data-end=\"2018\">Use of sedative drugs to control apprehension and\/or disruptive or pre-cooperative behavior<\/li><li data-section-id=\"15d2975\" data-start=\"2019\" data-end=\"2109\">Use of general anesthesia with an anesthesiologist to accomplish the necessary treatment<\/li><\/ul><p data-start=\"2111\" data-end=\"2296\">This treatment has been explained to me. Alternate methods of treatment, if any, have also been explained to me, as have the advantages, disadvantages, and risks of each. These include:<\/p><ul data-start=\"2298\" data-end=\"2574\"><li data-section-id=\"1a56i5k\" data-start=\"2298\" data-end=\"2346\">Pain and\/or sensitivity to temperature changes<\/li><li data-section-id=\"zowwq3\" data-start=\"2347\" data-end=\"2365\">Spontaneous pain<\/li><li data-section-id=\"1et2vzy\" data-start=\"2366\" data-end=\"2375\">Abscess<\/li><li data-section-id=\"13095nl\" data-start=\"2376\" data-end=\"2425\">Fracture of the tooth away from the restoration<\/li><li data-section-id=\"11bpbd8\" data-start=\"2426\" data-end=\"2471\">Partial or complete loss of the restoration<\/li><li data-section-id=\"muzld2\" data-start=\"2472\" data-end=\"2574\">Restoration failure and\/or loosening of the crowns necessitating their replacement or re-cementation<\/li><\/ul><p data-start=\"2576\" data-end=\"2833\">I am advised that though good results are expected, the possibility and nature of complications cannot be accurately anticipated and that, therefore, there can be no guarantee either expressed or implied, as to the result of the treatment or as to the cure.<\/p><p data-start=\"2835\" data-end=\"3227\">I recognize that during the course of treatment, unforeseen circumstances may necessitate additional or different procedures from those discussed. I therefore authorize and request the performance of any additional procedures that are deemed necessary or desirable to my child\u2019s oral health and well-being in the professional judgment of the Doctors of PEDIATRIC DENTAL CENTER OF GARDENS INC.<\/p><h2 data-section-id=\"50og2o\" data-start=\"3229\" data-end=\"3272\">Pediatric Behavior Management Techniques<\/h2><p data-start=\"3274\" data-end=\"3561\">It is our intent that all professional care delivered in our offices shall be of the best possible quality we can provide for each child. Providing a high quality of care can sometimes be made very difficult, or even impossible, because of the lack of cooperation of some child patients.<\/p><p data-start=\"3563\" data-end=\"3655\">Among the behaviors that can interfere with the proper provision of quality dental care are:<\/p><ul data-start=\"3657\" data-end=\"3849\"><li data-section-id=\"1gopcpf\" data-start=\"3657\" data-end=\"3672\">Hyperactivity<\/li><li data-section-id=\"xjm85k\" data-start=\"3673\" data-end=\"3694\">Resistive movements<\/li><li data-section-id=\"10ky049\" data-start=\"3695\" data-end=\"3751\">Refusing to open the mouth or keep it open long enough<\/li><li data-section-id=\"19k8pnz\" data-start=\"3752\" data-end=\"3849\">Aggressive or physical resistance (kicking, screaming, grabbing instruments or dentist\u2019s hands)<\/li><\/ul><p data-start=\"3851\" data-end=\"3992\">All efforts will be made to obtain cooperation using warmth, friendliness, persuasion, humor, charm, gentleness, kindness, and understanding.<\/p><h2 data-section-id=\"vr7la3\" data-start=\"3994\" data-end=\"4035\">Common Techniques Used in this Office:<\/h2><p data-start=\"4037\" data-end=\"4311\"><strong data-start=\"4037\" data-end=\"4054\">TELL-SHOW-DO:<\/strong> The dentist or assistant explains to the child what is to be done using simple age-appropriate terminology. Secondly, the child is shown the procedure on a model or on the finger of the dentist or assistant. Lastly, the procedure is performed as described.<\/p><p data-start=\"4313\" data-end=\"4489\"><strong data-start=\"4313\" data-end=\"4361\">POSITIVE REINFORCEMENT &amp; DESCRIPTIVE PRAISE:<\/strong> Praise is given to the child to reinforce cooperative behavior. Desirable behavior is acknowledged to encourage its recurrence.<\/p><p data-start=\"4491\" data-end=\"4612\"><strong data-start=\"4491\" data-end=\"4509\">VOICE CONTROL:<\/strong> A deliberate alteration of voice volume, tone, or pace to influence and direct the patient\u2019s behavior.<\/p><p data-start=\"4614\" data-end=\"4719\"><strong data-start=\"4614\" data-end=\"4630\">DISTRACTION:<\/strong> Diverting the patient\u2019s attention from what may be perceived as an unpleasant procedure.<\/p><p data-start=\"4721\" data-end=\"4897\"><strong data-start=\"4721\" data-end=\"4737\">MOUTH PROPS:<\/strong> A rubber or similar device is placed in the child\u2019s mouth to prevent closing and possible injury when a child refuses or has difficulty keeping the mouth open.<\/p><p data-start=\"4899\" data-end=\"5208\"><strong data-start=\"4899\" data-end=\"4961\">PHYSICAL RESTRAINT BY THE DENTIST\/DENTAL ASSISTANT\/PARENT:<\/strong> The dentist, assistant (under direction), and\/or parent may help restrain the child from movement by holding the child\u2019s hands, stabilizing the head, and\/or controlling leg movements or providing security by holding the child in the parent\u2019s lap.<\/p><p data-start=\"5210\" data-end=\"5464\"><strong data-start=\"5210\" data-end=\"5242\">PAPOOSE BOARD AND PEDI-WRAP:<\/strong> These are restraining devices used to limit disruptive movements and prevent injury, enabling the dentist to provide the necessary treatment. The child is wrapped in one of these devices placed on a reclined dental chair.<\/p><p data-start=\"5466\" data-end=\"5634\"><strong data-start=\"5466\" data-end=\"5495\">NITROUS OXIDE-OXYGEN GAS:<\/strong> The child remains conscious but may become drowsy or fall asleep. No sedation will occur without further explanation and specific consent.<\/p><p data-start=\"5636\" data-end=\"5844\"><strong data-start=\"5636\" data-end=\"5649\">SEDATION:<\/strong> Various drugs are used to relax a child who does not respond to other techniques or cannot cooperate due to age or maturity. Administered with Nitrous Oxide-Oxygen gas and with specific consent.<\/p><p data-start=\"5846\" data-end=\"6052\"><strong data-start=\"5846\" data-end=\"5869\">GENERAL ANESTHESIA:<\/strong> A medically induced state of unconsciousness allowing dental rehabilitation in a hospital, ambulatory, or dental office setting. Not provided without further explanation and consent.<\/p><p data-start=\"6054\" data-end=\"6170\">I have read and understand the listed pediatric dentistry behavior management techniques and alternative techniques.<\/p><p data-start=\"6172\" data-end=\"6281\">All questions about the behavior management techniques described have been answered in a satisfactory manner.<\/p><h2 data-section-id=\"ilk9ih\" data-start=\"6283\" data-end=\"6336\">Acknowledgment of Receipt of Information and Risks<\/h2><p data-start=\"6338\" data-end=\"6697\">I fully understand there is a possibility of surgical and\/or medical complications developing during or after the procedure. These risks and side effects may include adverse reactions to medication that could require hospitalization, additional surgical procedures, disability, systemic impairment, temporary or permanent nerve damage, brain damage, or death.<\/p><p data-start=\"6699\" data-end=\"6910\">I understand and have been informed that there are possible risks and complications associated with the administration of local anesthesia, sedation, general anesthesia, and medications. The most common include:<\/p><ul data-start=\"6912\" data-end=\"7172\"><li data-section-id=\"14tzk9\" data-start=\"6912\" data-end=\"6922\">Swelling<\/li><li data-section-id=\"y546aq\" data-start=\"6923\" data-end=\"6933\">Bleeding<\/li><li data-section-id=\"1j49vi6\" data-start=\"6934\" data-end=\"6940\">Pain<\/li><li data-section-id=\"kqusfl\" data-start=\"6941\" data-end=\"6961\">Nausea or vomiting<\/li><li data-section-id=\"1aayf4n\" data-start=\"6962\" data-end=\"6972\">Bruising<\/li><li data-section-id=\"14afxyw\" data-start=\"6973\" data-end=\"7044\">Tingling and\/or prolonged numbness of the lips, gums, face, or tongue<\/li><li data-section-id=\"1u1mu21\" data-start=\"7045\" data-end=\"7065\">Allergic reactions<\/li><li data-section-id=\"waytjy\" data-start=\"7066\" data-end=\"7087\">Hematoma (swelling)<\/li><li data-section-id=\"56yf98\" data-start=\"7088\" data-end=\"7098\">Fainting<\/li><li data-section-id=\"1vzfk7\" data-start=\"7099\" data-end=\"7172\">Lip and cheek biting resulting in ulceration or infection of the mucosa<\/li><\/ul><p data-start=\"7174\" data-end=\"7429\">I also understand there are rare but serious risks such as unfavorable reactions to medications, including respiratory and cardiovascular collapse (cessation of breathing or heart function) and lack of oxygen to the brain that may result in coma or death.<\/p><h2 data-section-id=\"1qn172m\" data-start=\"7431\" data-end=\"7477\">Authorization for Records and Use of Images<\/h2><p data-start=\"7479\" data-end=\"7629\">I authorize my pediatrician and\/or any other physicians or medical facilities to release any and all pertinent medical information regarding my child.<\/p><p data-start=\"7631\" data-end=\"7847\">I also authorize the Doctors at PEDIATRIC DENTAL CENTER OF GARDENS INC to use photographs, radiographs, diagnostic materials, and treatment records for the purposes of teaching, research, and scientific publications.<\/p><h2 data-section-id=\"1fmc6kl\" data-start=\"7849\" data-end=\"7888\">Financial Responsibility and Consent<\/h2><p data-start=\"7890\" data-end=\"8051\">I understand that the treatment plan and estimated fees presented may change depending on the time elapsed since the initial examination and the extent of decay.<\/p><p data-start=\"8053\" data-end=\"8301\">I accept full responsibility for payment of all services rendered, including charges not covered by insurance, state agencies, or other payers. I acknowledge I am also responsible for any collection fees in the event the account becomes delinquent.<\/p><p data-start=\"8303\" data-end=\"8490\">I authorize the release of pertinent information to individuals or organizations who may require it for the continuation of treatment, payment of services rendered, or credit referencing.<\/p><h2 data-section-id=\"iti88e\" data-start=\"8492\" data-end=\"8515\">Final Acknowledgment<\/h2><p data-start=\"8517\" data-end=\"8568\">I certify the accuracy of the information provided.<\/p><p data-start=\"8570\" data-end=\"8766\">I acknowledge that I have read and understood this consent. All questions regarding the recommended treatment, procedures, and behavior management techniques have been answered to my satisfaction.<\/p><p data-start=\"8768\" data-end=\"8924\">I understand I have the right to receive further clarification during the course of my child\u2019s care and that I have not been coerced into signing this form.<\/p><p data-start=\"8926\" data-end=\"9047\">I am aware that I may withdraw this consent at any time, and it will remain in effect until such withdrawal is submitted.<\/p><p data-start=\"9049\" data-end=\"9293\">I hereby authorize and direct the Doctors of PEDIATRIC DENTAL CENTER OF GARDENS INC to perform upon my child (or legal ward), if required, the necessary behavior management techniques to facilitate the provision of recommended dental treatment.<\/p><h1 data-section-id=\"1u65jir\" data-start=\"9295\" data-end=\"9328\">Member Financial Responsibility<\/h1><p data-start=\"9330\" data-end=\"9409\">Thank you for choosing PEDIATRIC DENTAL CENTER OF GARDENS INC as your provider.<\/p><h2 data-section-id=\"16adzcl\" data-start=\"9411\" data-end=\"9457\">Understanding Your Financial Responsibility<\/h2><p data-start=\"9459\" data-end=\"9723\">The dental services you seek imply a financial responsibility on your part. This responsibility obligates you to ensure payment in full for the services you receive. To assist in understanding that financial responsibility, we ask that you read and sign this form.<\/p><p data-start=\"9725\" data-end=\"9808\">Feel free to ask if you have any questions regarding your financial responsibility.<\/p><h2 data-section-id=\"1115ljy\" data-start=\"9810\" data-end=\"9834\">Your Responsibilities<\/h2><p data-start=\"9836\" data-end=\"9999\">You are ultimately responsible for all payment obligations arising out of your treatment or care and guarantee payment for these services. You are responsible for:<\/p><ul data-start=\"10001\" data-end=\"10137\"><li data-section-id=\"1grhula\" data-start=\"10001\" data-end=\"10014\">Deductibles<\/li><li data-section-id=\"r070ow\" data-start=\"10015\" data-end=\"10028\">Co-payments<\/li><li data-section-id=\"13r8srl\" data-start=\"10029\" data-end=\"10050\">Coinsurance amounts<\/li><li data-section-id=\"t4cbe9\" data-start=\"10051\" data-end=\"10137\">Any other patient responsibility indicated by your Children Dental insurance carrier<\/li><\/ul><h2 data-section-id=\"12uuygz\" data-start=\"10139\" data-end=\"10183\">Knowledge of Your Dental Insurance Policy<\/h2><p data-start=\"10185\" data-end=\"10323\">You are responsible for knowing your insurance policy. For example, you will be responsible for any charges if any of the following apply:<\/p><p data-start=\"10325\" data-end=\"10509\">I. Your Dental plan requires referral by a Primary Dental Physician (PDP) before receiving services at PEDIATRIC DENTAL CENTER OF GARDENS INC, and you have not obtained such a referral<\/p><p data-start=\"10511\" data-end=\"10579\">II. You receive services in excess of such authorization or referral<\/p><p data-start=\"10581\" data-end=\"10756\">III. Your Dental plan determines that the services you received at PEDIATRIC DENTAL CENTER OF GARDENS INC are not medically necessary and\/or not covered by your insurance plan<\/p><p data-start=\"10758\" data-end=\"10884\">IV. Your Dental plan coverage has lapsed or expired at the time you receive services at PEDIATRIC DENTAL CENTER OF GARDENS INC<\/p><p data-start=\"10886\" data-end=\"10941\">V. You have chosen not to use your Dental plan coverage<\/p><p data-start=\"10943\" data-end=\"11063\">If you are not familiar with your plan coverage, we recommend you contact your Dental carrier or plan provider directly.<\/p><h2 data-section-id=\"m89zrw\" data-start=\"11065\" data-end=\"11107\">Registration and Insurance Verification<\/h2><p data-start=\"11109\" data-end=\"11187\">You will be required to follow all registration procedures, which may include:<\/p><ul data-start=\"11189\" data-end=\"11354\"><li data-section-id=\"4r4y4i\" data-start=\"11189\" data-end=\"11233\">Updating or verifying personal information<\/li><li data-section-id=\"p98h4w\" data-start=\"11234\" data-end=\"11280\">Presenting verification of current insurance<\/li><li data-section-id=\"5j74j1\" data-start=\"11281\" data-end=\"11354\">Paying any co-pays or other patient responsibility amount at each visit<\/li><\/ul><p data-start=\"11356\" data-end=\"11446\">Your card or other insurance verification must be on file for your insurance to be billed.<\/p><p data-start=\"11448\" data-end=\"11582\">If we do not have your card on file, or are unable to verify your eligibility for benefits, you will be treated as a self-pay patient.<\/p><p data-start=\"11584\" data-end=\"11669\">As a self-pay patient, our fee is expected to be paid in full at the time of service.<\/p><p data-start=\"11671\" data-end=\"11858\">If the insurance card or other necessary information is furnished after the visit, we may file a claim with your insurance;
